ASSISTANCE WITH CARE AND HOUSING (ACH)

If you are a person 50 years or older who is homeless, or at risk of homelessness, then the Assistance with Care and Housing for the Aged Program (ACHA) can assist you to find the right accommodation for your needs.

This may include:

  • Assisting you to find housing through the private market.  
  • Assisting you to apply for affordable social
  • housing.
  • Assisting you if you are at risk of losing your present accommodation due to tenancy issues.
  • Supporting you to find a Residential Aged Care facility.

REFERRAL PATHWAY

You can be referred to the ACHA program through My Aged Care, Self, GP, other support networks and hospitals.
