Frequent Presentation Response Project (FPR)


In late 2015, Footprints secured funding from the Metro North PHN to provide a response to individuals with complex needs who frequently present to the Emergency Department of the Royal Brisbane and Prince Charles Hospitals and whose care can be better managed in the community. The FPR team aims to reduce patterns of dependence on Emergency Departments.The FPR Project encourages and assists individuals to gain access to the right combination of services, treatments, and specialist supports to better self manage their mental and physical health, sustain and maintain suitable tenancy and optimize capacity for inclusion in community life. The FPR model recognises the social determinants of health and the significance of community connection and participation. 

The FPR Team offers flexible service delivery, including after-hours and weekend support. It has already been recognised that individuals that contact emergency services regularly but are not transported to hospital will be a gap in service.
