The Resident Support Program supports older people and people with a disability who live in supported accommodation, boarding houses who are eligible to receive Home & Community Care Services in the Brisbane North area.

The Resident Support Program Outreach Worker can assist you to identify your health and support needs and help you to access services such as:
• Doctors
• Dentists
• Optometrists
• Podiatrists
• Personal Care
• Recreational/leisure activities
• Advocacy – mediation
• Information & referral

Support is flexible and appropriate to meet individual needs.

Anyone may contact Footprints to discuss services or refer someone to a program.
If you are contacting us on behalf of someone else, please ensure they are aware and you have their consent to do so.

Referrals
A medical referral is not necessary and referrals may be accepted from family, friends, community services, health professionals or you can contact us directly.
